Position: Community Follow-up Worker (CFW)

Location: Nassau or Suffolk County
Program: HIV/AIDS-Related Programs

Options' HIV/AIDS-related programs provide intensive case management, housing, and outreach services to low-income individuals and families with or at risk of HIV/AIDS.

Duties: The Community Follow-up Worker (CFW) assists the other members of the case management team (Case Manager and Case Management Technician) by having frequent client contact in the home and in the community. The CFW assists with scheduling of appointments, keeps simple records and participates in client case conferences. When necessary, the CFW escorts clients to ensure that appointments are kept.

Qualifications: High school diploma or equivalent required. Good communication skills and follow-up abilities. Knowledge of local community and resources are a plus.

Applicants are required to have a valid driver's license, access to a car, proper auto insurance, and a safe driving record. The employee uses his or her own vehicle for transportation between sites and is reimbursed for mileage.

Work Schedule: Monday - Friday 8:00am - 4:00pm OR
8:30am - 4:30pm OR
9:00am - 5:00pm
